水原性掻痒症（すいげんせいそうようしょう, 英: Aquagenic pruritus）は皮膚の症状で水との接触を原因とし、視認できる病変（英語）を伴わないものの、針で刺激するような強烈で重度のかゆみが特徴である[1][2]。水との接触が原因となるかゆみはじんましんを含め、水アレルギーとしても知られる。
病態[編集]
人によって異なり、散発的に発作を起こす人は10分から120分ほど持続し[3]、他の人は大気の湿度や発汗によりほぼ常に症状を示す。かゆみが最も頻繁に出る部位は脚、腕、胸、背中、腹部で、他の部位に発生する場合もある[4]。
病因[編集]
正確な病態の機構は不明。一部の研究によると、かゆみは皮膚の線溶系因子の増加[5][6]、交感神経系の異常な活性化[7]またはアセチルコリンエステラーゼ活性の増加に反応して起こると示唆される[8][9]。
診断[編集]
水原性そう痒症の医学的検査は未定である。診断はむしろ真性赤血球増加症を含め、かゆみの原因として想定できるすべての候補を減点法でしぼりこんで行われる。痒みは多くの深刻な疾患に見られる症状であり、最終診断を下す前に他の原因を除外することが重要である[10]。
治療[編集]
病原をすべての事例で完全に回避できないため、治療は通常、局所のかゆみ管理に重点を置く。液状もしくはクリーム状のかゆみ止めを塗る、光線療法、あるいはまた皮膚を水で濡らしてから温湿布や冷湿布をするなどで、かゆみが治る可能性がある[11]。温浴または温水シャワーで症状が軽減する患者が多いことから逆説するなら、おそらく熱刺激で皮膚のマスト細胞のヒスタミン供給が緩和し、その後、最大24時間にわたり分泌が止まると見られる[12][13]。
ロラタジン、ドキセピン、シメチジンなどのH1およびH2ブロッカーは、薬理学的治療において伝統的に最初の選択肢であるが、すべての患者がこれら薬物療法で症状の緩和を見るわけではない。抗ヒスタミン剤が有効な場合、ロラタジンは軽症例に最も効果的で、ドキセピンはより重症例にいちばん効果を発揮する傾向がある。
ナルトレキソン、ヒドロコルチゾンまたはプロプラノロールは、一部の人のかゆみを和らげる[14][7]。
セルトラリンまたは他の選択的セロトニン再取り込み阻害薬（SSRI）も一連の治療法に用いる可能性がある。
語源[編集]
名前はラテン語に由来し、水原は水が原因であること、掻痒はかゆみを指す。
